What is Form 1095-C?
Form 1095-C provides information about the health insurance coverage offered to you during the calendar year. You may need this form for your records or when preparing your federal tax return.

Who will receive a 1095-C?
You will receive a Form 1095-C if you were an employee of CHC who was eligible for benefits during the reporting year, even if you did not enroll in coverage.

Do I need this form to file my taxes?
The 1095-C is not required to be submitted with your tax return; however, you should keep it with your tax records.

To Access or Request a Paper Copy
Current employees may access in Paycor, by clicking on the W-2 tab on the home page or,
both current and former employees may request a paper copy of your Form 1095-C.
