Intake – Becoming a Client

Our programs are designed around the principles of trauma-informed care: safety, choice, collaboration, trustworthiness, and empowerment. At CHC, we meet our patients where they are, co-creating basic structure and support for their success. Participants work with staff to design an individualized plan to address sobriety, behavioral health, and personal goals.

How Do I Get Help?

Every patient starts with an intake/assessment at their first appointment. The goal of the Intake Department is to determine treatment needs and strengths to provide the appropriate level of care for each patient that walks in our door. The assessment process includes an interview, a medical appointment and urine drug screening. New patients will also meet with a representative from our financial department to review your insurance/ability to pay for services. We will work with you, regardless of your insurance status*

*Medicaid, Medicare, private pay and some commercial insurance plans are accepted. Summit County residents without insurance or underinsured are eligible for a sliding fee scale. CHC staff will help determine program eligibility and financial responsibility for each patient.

What to bring to your first appointment:

Valid state issued ID Insurance card (if you have one) Proof of income (i.e. last two pay stubs, if employed) Documents related to Medicaid/Medicare or SSI/SSSDI benefits (if applicable) If you are missing some of these documents, contact the intake department BEFORE your first appointment at 330-434-4141 and we can connect you with resources to help.
